At the age of 44, Talia Smith was a vibrant and athletic woman, dedicated to her fitness regimen that included daily workouts. However, following the use of a routine medication, her life took a drastic turn, leaving her reliant on a wheelchair and in need of palliative care at a hospice.

Talia’s life took such an unexpected and devastating turn after she went to her doctor for a urinary tract infection and was prescribed what she believed were routine antibiotics - now she needs 24-hour care to survive. “The doctor gave me Cipro,” she says. “I asked if there was anything I needed to know. They told me it was safe and effective. Three pills later, my life was turned upside down."

Ciprofloxacin, often referred to as Cipro, belongs to the fluoroquinolone family of antibiotics. In the UK, its prescription is now restricted to cases where no alternative antibiotic is suitable, due to the risk of severe side effects. Meanwhile, in the United States, the Food and Drug Administration (FDA) has issued seven black box warnings for this medication, representing the most serious warnings short of outright prohibition.

Talia's life took an unforeseen and drastic turn following what was supposed to be a straightforward appointment for a mild urinary tract infection.

However, Talia, a resident of Norwood, Massachusetts, was completely unaware that the medications she was using could lead to significant issues. "I was unable to walk, my whole body hurt, my eyesight was altered, and I had difficulty swallowing," Talia recalls. Concerned about her symptoms, she decided to go to the emergency room for assistance.

"The doctor inquired, ‘Have you been taking Cipro?’ I replied affirmatively, and that’s when he informed me about the black box warning. I was taken aback - I had never come across the term ‘black box warning’ prior to that moment."

Since 2008, the FDA has released alerts concerning fluoroquinolones, emphasizing the potential dangers including tendon ruptures, nerve damage, and muscle deterioration. The agency also advises that these antibiotics should only be utilized when no alternative treatment methods are viable, due to the lasting and debilitating side effects they may cause.

Talia expresses that her doctor never warned her about these risks. "What I experienced was like an explosion within me," she shares. "And it was all due to a medication I was assured was completely safe."

Talia's condition declined rapidly following her drug intake in April 2021. "I began to suffer from tremors, excruciating nerve pain, and stiffness that rendered me immobile. Just five months later, I found myself in hospice care," she recalls.

At her most challenging moment, Talia's weight dropped to a mere 4 stone 2 pounds, requiring constant care around the clock. "I couldn’t raise my arms, couldn’t eat on my own, and had to puree all my food," she shares. Her health issues made it impossible for her to look after her husband Tim, a disabled veteran who depends on a wheelchair. "It’s heartbreaking," Talia expresses. "I used to be the one taking care of everything, and now I find myself in need of care."

Resolved to transform her experience into a positive influence, Talia began sharing her story on TikTok, where she has attracted more than 1.4 million likes. Her candid and unedited videos chronicle her struggles and caution others about the possible risks associated with antibiotics. "I need people to be aware of the dangers," she explains. "If a doctor gives you a prescription, don’t take it without question. Verify, re-verify, and inquire. Your well-being is far too valuable to be left to fate. What I went through could happen to anyone."

Before Talia fell ill, she was the primary caregiver for her husband.

Her commitment to assisting others goes far beyond TikTok. In a groundbreaking development, the Centers for Disease Control and Prevention (CDC) has officially acknowledged "fluoroquinolone adverse effects," commonly referred to as "floxing" — a condition resulting from fluoroquinolone toxicity — as a reportable medical condition.

"This feels akin to a moon landing for us," remarks Dr. Stephen Pieper, a physician from Germany who has cared for more than 2,000 patients and collaborated closely with Talia and the project team, which includes the Fluoroquinolone Toxicity Study Foundation and patient advocate Audrey Reynolds.

The upcoming diagnostic codes, scheduled for implementation in October 2025, will enable physicians to formally record cases of floxing, facilitating insurance reimbursements, disability claims, and enhanced medical comprehension.

"When the announcement was made, I was flooded with calls, messages, and tears from people," Talia reveals. "They expressed their disbelief that this day had finally arrived." Although this recognition marks a significant advancement, Talia and her team remain committed to advocating for the incorporation of long-term floxing effects into the diagnostic standards.

Talia's struggles continued even after experiencing floxing. She now battles Mast Cell Activation Syndrome (MCAS), which was induced by the toxicity of fluoroquinolones. "MCAS is a nightmare," she explains. "I'm limited to just six foods, and I have to introduce any new items in tiny amounts to prevent severe reactions. Some days I can eat something without issue, and the next day it could trigger a reaction. It’s entirely unpredictable."

Her morning ritual showcases the obstacles she faces each day. "I have oatmeal each morning, enhanced with Camu Camu for vitamin C, since regular vitamin C doesn’t sit well with me. Even adding cherries has to be a gradual process," she explains.

Even though Talia faces numerous challenges, she is resolute in her mission to create change. "Dealing with MCAS is draining and lonely, yet I persist," she expresses. "Each day presents a battle, but I refuse to throw in the towel."

Her experience serves as a stark reminder of the risks associated with the excessive prescription of antibiotics and highlights the necessity for patients to be vigilant. “Ensure that you genuinely require an antibiotic before using one,” she advises. “And if a fluoroquinolone antibiotic is suggested, consider requesting a different option.”

Through her efforts, Talia aims to spare others from the trauma she faced. "We've accomplished something that will have a lasting impact on lives," she remarks about the newly implemented CDC codes.

"However, our journey doesn't end here; there's still plenty to accomplish." For Talia, each minor triumph - be it trying a new dish or witnessing floxing being acknowledged - serves as a proof of her strength and unwavering spirit.

"This journey has been the toughest challenge I've ever faced," she reflects. "However, if my experience can make a difference for even a single individual, then it has all been worthwhile."
